4. Eligibility & Patient Requirements

  • Age: Services are for adults 18+. Minors require parental/guardian consent.

5. Payment & Fees

  • Cash‑Only Practice: Payment due at time of service; we may furnish documentation for your insurer.
  • Accepted Methods: Cash, check (plus $35 fee for returned items), credit/debit card.
  • Fee Changes: New fees will not apply to pre‑paid appointments.
  • Hardship Arrangements: Available by prior agreement.

6. Cancellation & No‑Show Policy

  • 24‑Hour Notice: Required to avoid a fee.
  • Late Cancellation (< 24 hrs): 50% of scheduled service fee.
  • No‑Show: 100% of scheduled service fee.
  • Repeated No‑Shows: May lead to termination of care.

7. Telehealth Terms

  • Technology: You’re responsible for reliable internet, video/audio capability, and privacy.
  • Limitations: Not for emergencies, physical exams, or initial diagnoses of serious conditions.
  • Prescribing: Controlled substances require in‑person evaluation.

8. Website Use & Restrictions

  • Permitted: Learning about Services, scheduling, accessing resources, designated communications.
  • Prohibited: Unlawful use, unauthorized access attempts, distribution of malicious code, content scraping, harassment, impersonation, or copyright infringement.

9. Intellectual Property

All Website content—text, graphics, logos, images—belongs to the Practice or licensors. You’re granted a limited, non‑exclusive license to view for personal, non‑commercial use. Unauthorized use is prohibited.

10. User‑Generated Content

17. General Provisions

  • Waiver: Failure to enforce any provision is not a waiver.
  • Assignment: You may not assign your rights; we may assign ours.
